They're one problem, surfacing in seven places: no shared data both teams can see, and no defined handoff for the moments information needs to cross from one team to the other.
Seven symptoms and one cause is a much easier problem to fix than seven separate ones.
How this actually gets fixed
Two structural pieces close most of this gap: a real handoff process for the moment a deal closes, and a shared source of truth both teams look at on an ongoing basis afterward. Neither is a meeting. Both are things a system can enforce instead of relying on someone remembering to communicate.
If more than one or two of these sound familiar, that's usually a strong enough signal on its own — worth a proper look rather than another attempt at better communication.
